Solution The preferred method is electronically. To do this, the form must be signed by the CEO and scanned into Adobe PDF format. Then submit the form and all required document through the AVC feature on this website.

If you do not have the capability to scan documents and want to mail them, please contact the accreditation center at 804-786-8421
